Requirements
1. Accomplished Pre-Baptismal Form

2. Photocopy of the Child's Birth Certificate
(If the PSA Birth Certificate is not yet available, a copy of the Certificate of Live Birth with Registry number may be submitted instead.)
3. For Adult Baptism (7 years old above) with residency within BGC, McKinley Hill, Minipark, & MacArthur.
(Kindly set an appointment with our Baptism Secretary for a detailed discussion on the requirements needed.)

Attire
1. For the Baby - All white clothing with white cap
2. For the Parents and Godparents - Please wear proper/presentable clothing or a “Sunday’s Best” attire.
General Reminders
Kindly pencil book your preferred date and time of Baptism through personal appointment at our Baptism Office.
Pencil bookings are on-hold for seven (7) days and will be removed and opened to other parishioners if no reservation fee is settled.
Please complete all the necessary requirements at least SEVEN (7) DAYS before the actual Baptism schedule to avoid any conflicts and delay in recording for the Canonical Book of Baptism.
For Solo Baptism, please be at the church thirty (30) minutes before the actual Baptism schedule to avoid any delays.
Baptismal candles and booklets will be prepared and provided at the Baptistry Room (or at the Main Church Area for Communal Baptism.)
Baptismal Certificates will be available two (2) weeks after the baptism schedule.
